Dog Boarding Intake Form

The following is for your information.

Completing this form is the first step in assessing your dog's suitability to come into care, and is also dependent on our availability, and does guarantee acceptance into care.

Prior to coming into care, all new clients will be required to complete the following:

Review, sign in acceptance, and return, our standing boarding agreement / waiver. This only needs to be done once, unless there are changes in your information.
Book a 3 hour trial visit, by email
If successful, book a 24 hour trial visit by email.

These are paid services.

Once your dog is fully accepted to come into longer term care, and in order to secure your dog's spot, we require a 50% deposit. The balance is due, on or before dropping your dog off.

We invoice by each 24 hour period, so drop off and pick up needs to be at the same time. As you likely know, my work schedule is variable, so please be prepared to be flexible while working to find a mutually suitable time.

PRIOR to your dogs stay, emails with additional information, and also to gather details about your dogs needs while in care will be emailed to you separately.

Depending on how long since you originally completed our behaviour intake form, which included vaccination records, we may require updated vaccination records prior to coming into care.

Completing this form is only required once. Future boarding needs can be requested by email.

Do you have any other current specific behaviour concerns? If Yes, please explain.

Does your dog have any health problems that we should now about? Including allergies or food sensitivities of any kind.

Will your dog need medication or treatment of any kind while in care.

We separate dogs during meal times, for resting periods, when it's working on a chewy. Is your dog comfortable in a room behind a gate? (door open).(To select multiple, hold Ctrl or Command)

Is your dog comfortable being left at home alone for up to 3 hours at a time?

When was the last time your dog had an 'accident' in the house? Pee or Poo.(To select multiple, hold Ctrl or Command)

To eliminate transmission, we require that dog's coming into care be treated for fleas, ticks and parasites.(To select multiple, hold Ctrl or Command)

Is there anything else of importance that you'd like to share.

All dogs coming into care are required to be fully vaccinated, or have been titre tested. If your dog's vaccine status has been updated since you last provided it to us, please attach updated records.
